Dead cockroaches were found accumulating in the dining room and the back storage room next to the office. The facility lacked a designated certified food protection manager on staff. A nonfood-contact surface in the chemical storage room was found to be made of absorbent synthetic wood. Missing tiles, cracks and small holes were observed in the walls and floors of employee restrooms and storage rooms. Time/temperature control for safety (TCS) foods, including sliced carrots, onions, cabbage, cucumber and dressing, were measured at temperatures above 41 degrees. Substantial soil and debris buildup was found behind stove tops in the kitchen and underneath booths in the dining area. The kitchen ceiling and floor/wall junctures were not smooth or easily cleanable due to damage and missing tiles. The establishment failed to maintain adequate temperature logs for cooling cooked shrimp and meat. There were no contemporaneous calibration logs available for the facility鈥檚 temperature-measuring devices.

Multiple piping systems throughout the bar were found to have inadequate air gaps into the floor drains, creating a risk of backflow contamination. Containers of salt, sugar and spice mix located above the bar top were not labeled with their common names after being removed from original packaging.

Econo Lodge, 2321 Central NW

The person in charge was unable to demonstrate knowledge regarding reportable foodborne illnesses or the difference between time and temperature as public health controls. An inadequate air gap was observed under the three-compartment sink, which is necessary to prevent backflow contamination. The food establishment did not have a certified food protection manager on staff as required. Food employees did not have valid food handler cards or other approved food safety training. The facility lacked a chlorine testing kit or strips to measure the sanitizer concentration in the warewashing machine. No handwashing signs were posted at the sinks located in the kitchen area. A handwashing sink in the kitchen was being used for storage, specifically for holding gloves inside the basin. The facility was found to be operating with an expired food permit. Soil residues and debris had built up on the exterior of the ice machine. The kitchen area was in disrepair, characterized by holes in the ceiling and missing vent covers. The establishment failed to maintain required temperature logs for equipment, hot and cold holding, cooling and warewashing. No logs were available to document the calibration of temperature-measuring devices.

Ladera Nursing & Rehab Center, 5901 Ouray NW

Debris and soil buildup were found on the ice machine chute and can opener, while an employee was observed handling clean dishes immediately after touching soiled ones. Working containers for soap and sanitizer were not properly identified with common names, and an employee was found using a green soap bucket for sanitizer. Multiple TCS food items, including eggs, salsa, dressing and green chile, were found in the walk-in cooler without date markings or with shelf lives exceeding seven days. Polypropylene cutting boards on the dry storage rack were found with deep scratches and scoring that prevented effective cleaning. An employee was observed performing multiple tasks and touching various surfaces without changing single-use gloves between activities. The flow pressure measuring device on the mechanical warewashing machine was damaged, with bent arrows and non-functional gauges. The person in charge failed to coach team members on handwashing and was unable to demonstrate proper knowledge of food safety regulations. The plumbing at the prep sink was leaking sewage or grey water onto the floor, and a pipe was found unattached. An inadequate air gap was observed at the prep sink, creating a risk of water supply contamination. The outside waste storage area was found unclean, with food and liquid waste leaking from the bulk bin. An employee was observed wearing multiple unapproved bracelets while handling and preparing open TCS foods. A food employee was observed handling open TCS foods in the kitchen without proper hair restraints. Significant buildup of debris and grey water/sewage was observed on the walls and floors, particularly in the kitchen area. Significant debris buildup was found on shelving in the walk-in cooler and on the sides of various kitchen equipment and plumbing. Numerous gaps, cracks, and holes were identified in the walls and ceilings of the kitchen, dry storage and mop sink areas. The flooring grout at the prep sink was damaged or missing, resulting in pooling liquid and trapped food debris.

Peter Piper Pizza, 3109 San Mateo NE

Ice buckets, a vegetable dicer and a shredding robocoupe were stored away without being properly washed, rinsed and sanitized. The facility lacked written procedures for using time as a control for TCS foods on the buffet line. The establishment did not have a certified food protection manager on staff at the time of inspection. The hood ventilation system above the frying area and pizza oven was inadequately collecting grease and condensation, leading to drips on equipment surfaces. Mixed salads on the buffet line were measured at temperatures between 48 degrees and 53 degrees, exceeding the 41-degree limit. Ice was exposed to potential contamination because the top sliding door of the ice machine was missing. No testing kits or devices were available to measure the concentration of chemical-sanitizing solutions. The lighting in the walk-in refrigerator was inadequate for conducting safe operations and thorough cleaning. A food employee was observed preparing dough with a large beard and no hair restraint. The facility failed to display the grade sticker from its most recent inspection in a place visible to the public. An employee was observed donning new gloves and switching between cashier and food handling duties without washing their hands. Debris and soil buildup were found underneath cold-holding units, prep tables and storage shelving. Substantial soil and grease buildup was found on kitchen shelving, fryers and inside the dough storage reach-in refrigerator. The establishment failed to maintain temperature logs for stored and received cooked meats, vegetables and dairy products. No logs were available to document the regular calibration of temperature-measuring devices.
