Quaternary ammonium solution used to sanitize food-contact surfaces failed to meet parameters to adequately achieve sanitization. The facility was using floor cleaner as a sanitizer in a sanitation bucket. There was debris buildup on the blade where the can opener opens cans. Dead beetles were located on the wall near the ice machine and the wall near the electrical panel in the back area. Ready-to-eat TCS food was prepared and held within the establishment with no date-marking indicators. The food establishment does not have a certified food protection manager on staff. There was no documentation or expiration date inside the tube-testing kit or other device to measure the concentration of chemical sanitizing solution. The food establishment does not have temperature logs available for TCS food items. The food establishment does not have calibration logs for temperature-measuring devices.

Heating and AC system vents were not properly designed or located to be easily cleanable. The hood ventilation system was inadequately collecting grease and condensation, allowing for accumulation to occur. Chemical sanitizer concentration for wiping cloth containers exceeded the maximum level permitted. Chlorine solution used for the automatic dish machine failed to meet the parameters to adequately achieve sanitization. TCS food items, specifically cooked shredded chicken, were cooling in a method that did not facilitate the necessary rate of heat transfer. Ready-to-eat food items, including red chile meat, green chile sauce, refried beans, soup and tamales, were held within the establishment with no date-marking indicators. The facility did not provide a designated area for employees to eat, drink or use tobacco products. Food items that were not unmistakably recognized, such as possible brown and white sugar, were removed from the original packaging without proper labeling or identification. Poisonous or toxic materials, specifically rubbing alcohol, were stored in a manner that had the potential to contaminate food. A food employee did not wash their hands when required, such as when switching job duties between taking orders and preparing food. The only handwashing sink inside the kitchen was blocked by a pitcher stored inside the sink basin. A TCS food item, specifically a tamale, was not hot held at 135 degrees or above. The back door to the outside was propped open with no barrier to prevent insects, rodents and other animals from entering the kitchen. Nonfood-contact surfaces, including prep tables, fryers, shelving and a bus cart, had a buildup of soil residues. Debris and buildup were observed on the flooring underneath the fryer, stove top and other kitchen units. Fruit cooling for aguas frescas was not protected from contamination by being covered or wrapped while stored next to a handwashing sink. Food employees did not have a food handler card or were not trained in an approved food safety course. The food establishment did not have temperature logs available for TCS food items.
