Numerous rodent droppings were found throughout the property. A rodent nest with a dead mouse was found inside the nesting material. Water leaked profusely from the top of the ice machine. Organic growth was present on the ice machine panels and piping. A plastic cutting surface had deep scratches and scoring. Service counter shelving was improperly installed and had unsealed gaps. Rodent droppings were located on and under the service counter shelving. The floor drain lacked an adequate plumbing air gap. An untreated wood surface had spillage and droppings present. Outside doors were not tight-fitting and showed large gaps. A screen door was damaged, allowing potential pest entry. The swamp cooler and electrical panel box had unsealed openings into the structure. Dirt and debris built up on floors and walls behind equipment. The warewashing machine had 0 ppm chlorine sanitizer concentration. Food containers and cooking utensils had heavy debris and grease buildup. Employees used an unapproved Raid pesticide spray inside the kitchen. Raw meat was thawed improperly in a sink basin without running water. Soap splattered onto the raw meat during dishwashing. An active cockroach infestation was observed throughout the facility. Live cockroaches crawled directly on onions and carrots meant for service.
